So this is new. In updating to Android 11 they seem to have removed the don't ask again option whenever you plug in any USB DAC that also exposes an input. Anyone know of a way for this to go away?

There's a 'Chat' and 'Audio' device that works independently of each other. NMP attaches to the 'Chat' one presumably because that's the first one that enumerates when the USB is inserted. There is no option for Exclusive access, is it the same as the 'Direct USB access' option? It is already on and there is no impact on any of the settings. Only Stereo L R is listed unless the generic driver is on.

Yes, speaker output is active when connected. The headunit seems to be decoding a file when audio is playing, Bluetooth can be off and it still plays. It shows up as OpenSL ES Speaker, so I guess setting it to use the speaker should work? It makes the phone speaker unusable but that isn't an issue for me in particular.
